During breast augmentation under general anesthesia, the surgeon first makes a small incision, lifts the breast tissue, and creates an implant pocket. The breast implant is then securely inserted into this pocket. The surgeon then positions the implant either partially or completely under the pectoral muscle. This submuscular implantation is particularly preferred for very slender patients with little fat or glandular tissue. Alternatively, the implant can be placed under the mammary gland and above the pectoral muscle. This subglandular implantation leaves the breast tissue largely unchanged and is suitable for patients with sufficient existing breast tissue. The necessary incision is made in such a way that as little as possible remains visible after the surgery. Typical access points are the inframammary fold, the area around the areola, or the armpit. For saline implants, access can also be gained through the navel. A special method is endoscopic breast augmentation via the armpit. A thin endoscope is inserted behind the pectoral muscle through a small incision in the natural armpit crease. The muscle is gently released to ensure natural breast movement during arm movements. The magnified endoscopic image allows for precise control. This enables very accurate shaping of the implant pocket and minimizes the risk of injury to sensitive nerves. Both round and anatomical breast implants up to a size of 480 cubic centimeters can be safely inserted. In the hands of an experienced surgeon, this technique is just as safe and precise as the classic incision under or on the breast. The operating time is often shorter. However, there is no universally preferred method. Which approach is suitable always depends on the individual anatomy and the aesthetic goals of the patient.
